Mathura, November 2, 2026: In Mathura, the procedure for obtaining a disability certificate and a UDID card has been streamlined and is now entirely conducted online. According to administrative guidelines, applicants must use the Government of India’s official Swavlamban portal to initiate their request for these credentials. The shift to a digital-first system is intended to improve accessibility and transparency for residents seeking disability services.

Required Documentation and Portal Access

To begin the application process, citizens must visit the official website at swavlambancard.gov.in and select the option for “Apply for Disability Certificate & UDID Card.” Preparation of necessary documentation is a prerequisite for a successful submission. Applicants are required to provide a valid Aadhaar card, which serves as proof of both identity and address. Additionally, users must upload a recent passport-sized color photograph along with a specific photograph showing the affected limb. Any previous medical reports or hospital prescriptions from ongoing treatments should also be kept ready. The application also requires an active mobile number and the applicant’s digital or scanned signature or thumb impression.

Step-by-Step Submission Process

When filling out the form on the portal, users must input their personal details and residential address accurately. A critical step involves choosing the specific category of disability from a list of 21 recognized types. The system will default to the district government hospital for Mathura, which the applicant must select to finalize their registration. Once all details are verified and documents are uploaded, the form is submitted. Upon successful submission, the system generates an enrollment number or receipt. It is essential for the applicant to download and retain this receipt, as it is mandatory for the subsequent medical evaluation phase.

Medical Evaluation and Certification

The medical examination portion of the process is conducted by a designated medical board at the District Hospital, Mathura or the office of the Chief Medical Officer (CMO) in Mathura. The examination can only take place after the online application has been submitted. Applicants must visit the facility with their original documents, the printed enrollment receipt, and any relevant previous medical records. The medical board performs the assessment to determine the percentage of disability. To be eligible for a formal disability certificate, the determined level of disability must be at least 40 percent. Following the assessment, the doctors verify the online application against the physical examination results and update the findings directly onto the digital portal. The certificate is then generated online once the process is complete.

Fees and Service Charges

The official application and certification process provided by the government through the UDID portal is free of cost. However, for residents who seek assistance at a Jan Seva Kendra (CSC) or a local cyber cafe, there may be a service charge. These centers typically charge between ₹15 and ₹50 to assist with the online form entry. Residents are advised to prioritize the online application on the official portal before seeking in-person medical verification, as the system does not facilitate clinical appointments without a prior digital submission.
